PROBLEM: The Texas Medicaid Managed Care (TMMC) project is a public health managed care initiative in the North Texas Region. The community this proposal seeks to serve encompasses the most rural areas of this region.
Currently, very little coordination exists between the STAR and NorthSTAR programs. Fragmentation between these systems of care translates often to little or no coordination of services for children and adolescents needing medical and behavioral health services. GOALS AND OBJECTIVES: The purpose of this proposal is a vision of a truly integrated system of care. The goals of this project are:
1. Develop a comprehensive model for an integrated system of care for NorthSTAR East incorporating medical, mental health, and substance abuse service systems for children, adolescents and their families.
2. Develop an implementation plan for the integrated system of care model.
3. Develop a Performance Management System (PMS) to provide on-going evaluation of the effectiveness of the project and the resultant integrated system of care.
